9.1.2– Accident and Incident Report Procedures
Standard: There shall be established procedures for accident and incident reporting and analysis of accident and incident reports. There shall be an accident/incident report form available to all employees. Data shall be obtained in an appropriate manner to support planned and coordinated accident prevention programs within the agency.
Suggested Evidence of Compliance: Provide the accident/incident form(s) and the procedures for documenting and analyzing accidents and incidents. The accident/incident form(s) shall include identification information (who), specific location of accident (where), and description of accident in terms of sequence of activity (what).
COMMENT: Need: Provide procedural steps for Documenting AND Analyzing accidents and incidents (e.g., Procedure, Manual, Training). step-by-step manual on how team members should create and submit these forms for review.
RESPONSE: Accident/Incident-Reporting Policy & Procedures can be found in sections 401 & 402 of the PRC Administration Manual (page 38). This Manual is now included as EOC 6. The PRC real-time incident notification policy is further included as EOC 7 and the email updating staff of this policy is also included as EOC 8.

Narrative:
Public and employee accidents, incidents and illnesses are reported in a timely manner by PRC facility staff at which they have occurred. In an effort to streamline and centralize reporting for incidents, Westchester County (WC) provides an Incident Tracking Form and Employee Injury & Illness Incident Report which are accessible on the PRC Intranet. They allow assigned users to fill in information on who, what, when, where, and subsequent action taken, such as need for ambulance transport.
This system allows for the Risk Management Analyst to identify trends and patterns to further facilitate health and safety. By allowing for a search of prior incidents to determine whether a pattern exists because of physical conditions, the Analyst can contact the County Department of Public Works and Transportation to provide for and implement a solution to help eliminate risk of recurrence.
PRC also follows all applicable New York State procedures.
